Contact information for key programs and services at this installation.
Health care services provided by the Defense Department are available to you and your family at your installation. We know that finding the right health care is important, especially in special circumstances such as emergency or during recent move. Here, you’ll find information and options about the services you have regarding healthcare.
The 366th Medical Group provides medical services to support the combat capability of the 366th Fighter Wing at deployed locations as well as the Mountain Home Air Force Base community. Clinics provide dental, family health, flight medicine, pediatrics, immunization, and optometry with specialty care services in the following: women’s health, radiology, pharmacy, clinical laboratory, mental health, and physical therapy. Specialty services beyond the capability of the 366th MTF are referred to network facilities in the surrounding area.
When you move, TRICARE moves with you. No matter where you go in the world, TRICARE is there before, during and when you get to your next duty station.
There are steps you need to take to make sure changes to your TRICARE coverage go smoothly. Understand that when you move, you may have to use a different TRICARE option. Visit TRICARE Moving to learn more.
Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.
Filling prescriptions while traveling
You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.
Getting dental care while traveling
Getting dental care while traveling depends on your location and whether you are a service member or family member.
With permanent change of station orders, ask for a copy of your medical and dental records from your military treatment facility and the dental treatment facility. Do this at least one month before your PCS date. The MTF should also transfer a copy of your record and any family records to your new duty station or you may be able to hand carry them to your new duty station.
If you want someone else to be able to get medical or dental information on you or your family while you are moving, you need to complete a DD Form 2870, "Authorization for Disclosure of Medical or Dental Information."
For more information on getting copies of medical records, visit TRICARE Request Copies of Medical Records.

Dental Care
Emergency Care -- For emergencies, sick call is open to all military, family members, retired and family members of retired personnel. Acute problems such as pain, bleeding and injuries to the mouth are considered emergencies. For true emergency conditions which occur outside normal duty hours, patients should report to the hospital urgent care room. Non-active duty patients, however, should seek emergency care directly from their civilian dentists whenever possible.
Active Duty Personnel -- Comprehensive general dentistry is provided at this hospital. Types of dental treatment include examination, cleaning, all types of fillings, crowns, bridges, dentures, extraction of teeth, root canal therapy, treatment of gum disease, some tooth bleaching and biopsies of small benign growths. Individuals who know they need dental treatment should contact the dental clinic for an appointment at 208-828-7300.
Family Members, Retirees and Retiree Family Members -- Routine dental care is not available except to active duty members. Appointments will not be made for other than active duty patients. On dental sick call, only emergency treatment will be given. Sponsors must take the responsibility to provide for dental care for their family members.
TRICARE Family Member Dental Plan (TFMDP) -- The TRICARE Family Member Dental Plan (TFMDP) is a comprehensive and affordable dental plan available to eligible spouses and children of active duty Uniformed Service members. The TFMDP has cost shares, benefit maximums and coverage limitations similar to typical group dental insurance plans.
To be eligible for TFMDP enrollment, sponsors must intend to remain on active duty for at least 24 months and this commitment must be confirmed by their parent Uniformed Service. Enrollment and/or changes to enrollment can be accomplished by filling out a DD Form 2494 or 2494-1 at the sponsor's servicing personnel office. These forms must be read carefully as they provide detailed information on enrollment options, limitations and when TFMDP coverage begins.
Upon enrollment, the sponsor must pay the monthly premium through their Uniformed Services payroll account - the Government pays 60% of monthly premiums. Before providing care to family members, some dental providers may ask to see a copy of the sponsor's Leave and Earnings Statement showing the dental plan payroll deduction.
Coverage is worldwide and portable between assignments. Metlife Inc. administers the TFMDP. Detailed TFMDP coverage and enrollment information can be found in the TFMDP benefit booklet. For more information about the program, visit UCCI's web site or contact them at 1-855-638-8371 CONUS for information in the TFMDP in the United States, Canada, Guam, Puerto Rico and the U.S. Virgin Islands, or 1-855-638-8372 for information on the TFMDP in all other locations.
