Health care services provided by the Defense Department are available to you and your family at your installation. We know that finding the right health care is important, especially in special circumstances such as emergency or during recent move. Here, you’ll find information and options about the services you have regarding healthcare.
When you move, TRICARE moves with you. No matter where you go in the world, TRICARE is there before, during and when you get to your next duty station.
There are steps you need to take to make sure changes to your TRICARE coverage go smoothly. Understand that when you move, you may have to use a different TRICARE option. Visit TRICARE Moving to learn more.
Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.
Filling prescriptions while traveling
You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.
Getting dental care while traveling
Getting dental care while traveling depends on your location and whether you are a service member or family member.
With permanent change of station orders, ask for a copy of your medical and dental records from your military treatment facility and the dental treatment facility. Do this at least one month before your PCS date. The MTF should also transfer a copy of your record and any family records to your new duty station or you may be able to hand carry them to your new duty station.
If you want someone else to be able to get medical or dental information on you or your family while you are moving, you need to complete a DD Form 2870, "Authorization for Disclosure of Medical or Dental Information."

374th Medical Clinic
The 374th Medical Group's clinic provides quality care for all beneficiaries, and is located on the East side of the base. It offers a wide range of acute, routine, preventive and specialty services. Patients who require services not available at Yokota Air Base are referred to local Japanese hospitals or the closest military treatment facility with the required specialty care.
Each squadron on Yokota is assigned to a Primary Care Management team. The TRICARE Service Center can verify which PCM team you are assigned to. TRICARE can be reached at DSN 315-225-5036.
All primary care appointments are scheduled by the PCM team appointment clerks, who can be reached at DSN 315-225-8864. From your cell phone, call the base operator at 042-552-2510, then enter the last five digits 5-8864, for the appointment line. The Dental clinic can also be reached at this number.
All routine medical care begins with your PCM. If you are an Airman, you should contact Flight Medicine. If you are not a flyer, contact the Primary Care Clinic. If any specialty care is needed, you must be referred by your PCM. Patients needing emergency and/or urgent care after normal duty hours can be seen in the Urgent Care Clinic on a walk-in basis, 24 hours a day. If you are on base and require emergency services, call 911. Yokota emergency services cannot respond off base.
Yokota Pharmacy
The Yokota pharmacy offers over the counter medications following consultation with a pharmacist to evaluate symptoms. The intent of the OTC clinic is treat ACUTE symptoms associated with seasonal allergies, aches/pains, and cough/cold/fever. The OTC program can be utilized once every 90 days. Patients needing more frequent treatment must consult their provider for a recurring prescription.
