When you move, TRICARE moves with you. No matter where you go in the world, TRICARE is there before, during and when you get to your next duty station.

There are steps you need to take to make sure changes to your TRICARE coverage go smoothly. Understand that when you move, you may have to use a different TRICARE option. Visit TRICARE Moving to learn more.

Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.

Filling prescriptions while traveling

You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.

Getting dental care while traveling

Getting dental care while traveling depends on your location and whether you are a service member or family member.

• If you are an active-duty service member within the United States or in U.S. territories, and near a military installation, you may be able to get emergency dental care at the dental treatment facility, or DTF, if available. If not, contact the TRICARE Active Duty Dental Care Program before you see a dentist and tell the contractor about your move. You will need to get an authorization before you see a dentist. The Active Duty Dental Care Program will work with you, so you can get care from a local network dentist.
• If you are an active-duty service member at an overseas location, call the TRICARE overseas regional call center to get authorization before you see a civilian dentist.
• If you are an active-duty family member within the United States, in U.S. territories or at an overseas location, you can find out if any nearby military dental treatment facility treats active-duty family members. If you're enrolled in the TRICARE dental program, call the TRICARE dental program contractor or visit their website to find a provider. You will need to get an authorization before you see the dentist.

374th Medical Group Clinic

The 374th Medical Group's clinic, located on the east side of the base, provides quality care for all beneficiaries, and offers a wide range of acute, routine, preventative, and specialty services. Patients who require services not available at Yokota Air Base are referred to local Japanese hospitals or the closest military treatment facility with the required specialty care.

Each squadron on Yokota is assigned to a Primary Care Management team (PCM). The TRICARE Service Center can verify which PCM team you are assigned to. TRICARE can be reached at DSN 315-225-5036.

The PCM team appointment clerks schedule all primary care appointments, which can be reached at DSN 315-225-8864. From your cell phone, call the base operator at 042-552-2510, then enter the last five digits 5-8864 for the appointment line. The Dental clinic can also be reached at this number.

All routine medical care begins with your PCM. If you are a flyer, you should contact Flight Medicine. If you are not a flyer, contact the Outpatient Care Clinic. If any specialty care is needed, your PCM must refer you. Patients requiring emergency and/or urgent care can be seen in the Urgent Care Unit on a walk-in basis, 24 hours a day. If you are on base and require emergency services, call 911. Yokota emergency services cannot respond off base.

Yokota Pharmacy.

The Yokota pharmacy offers over-the-counter (OTC) medications after a pharmacist consultation to evaluate symptoms. The OTC clinic intends to treat ACUTE symptoms associated with seasonal allergies, aches/pains, and cough/cold/fever. The OTC program can be utilized once every 90 days. Patients needing more frequent treatment must consult their provider for a recurring prescription.

Exceptional Family Member Program - Medical (EFMP-M)

The EFMP-M is in the Main Hospital and can be reached at DSN 315-225-6233. The EFMP-M is responsible for the following:

1. Identification and enrollment
2. Family member travel screening
3. Command sponsorship
4. EFMP-M re-assignments
