Services are provided by appointment only:
Wills, living wills, and healthcare power of attorney – Legal assistance attorneys can prepare wills and advance medical directives, including living wills and medical powers of attorney.
Before your appointment, visit the Air Force legal assistance website to fill out a will worksheet form.
When you have completed the form, you will receive a ticket number. With this ticket number, call DSN 315-225-8069 or commercial 042-552-2510 xt 5-8069 to make your appointment.
