Legal assistance is available to you and your family. Below, you’ll find information about claims and installation specific programs and services. If you have questions, reach out to the legal assistance program at your installation.
Legal Assistance
Assistance is provided by appointment for Active Duty, dependents, and retirees. We provide informational pages titled Legal Lines on a whole host of legal topics at the Legal Office. The Air Force has also instituted a legal assistance website to aid in obtaining legal assistance. Call 501-987-7886 for more information or schedule and appointment.
Powers of Attorney and Notary Services
Walk-in service (no appointment necessary) is available Monday through Friday from 8:30 a.m to 4:30 p.m and Wednesdays 8:30 a.m to 2:00 p.m. The legal assistance website also aids in helping to prepare Powers of Attorney prior to coming into the office. This website can be accessed on any public computer; all you would need to have is your ticket number when coming to the Legal Office to receive your Power of Attorney.Will Executions
Wills are provided on an appointment basis only and are drafted and executed in a single appointment. Appointments are offered for Active Duty, dependents, and retirees every Thursday at 1:00 p.m. The will worksheet is available on the legal assistance website, just ensure that you bring in the ticket number which is located at the end of the questionnaire or you can contact the Legal Office directly where they will able to send you the PDF version. Couples are welcome to have an appointment together, just keep in mind that you will be slotted for a two hour window and will require two separate will worksheets and ticket numbers. Filing Household goods claims is now online.
The Air Force Judge Advocate General's Corps has consolidated the processing of certain claims, commonly known as personal claims, at a single location in Dayton, Ohio.
These claims involve loss or damage during shipment of household goods and privately owned vehicles as well as during storage at government expense, and loss or damage to personal property located in government quarters or other authorized locations incident to government service.
Other types of trauma-related claims, such as those resulting from the alleged negligence of a government vehicle driver, for example, will continue to be processed at the base legal office.
For personal claims, the new process will enable Air Force employees to file claims on-line and electronically submit required documents to the Air Force Claims Service Center (AFCSC) in Ohio. Employees can complete the process from home after a one-time login from any government computer, or stop in at the Legal Office to utilize the scanner and computer that is provided for this reason. If you don’t have access to a government computer, call the claims center at DSN 312-986-8044 or 937-656-8044 and they will provide a username and password that is good for 60 days.
As employees complete their claims on-line, they can get customer service over the Web or from the center by phone. Not only will the on-line filing system accelerate the processing of the claim, it will allow for quicker payment.
Full Replacement Value (FRV) Claims
To file for Full Replacement Value, you will need to file the claim directly with the carrier keep in mind a few things:
For general information on filing a claim, go to the claims website or call the base legal office. To file a claim, go to the website or call DSN 312-986-8044 or 1-877- 754-1212 with questions.
1) Online Filing -- After gathering all the required paperwork, proceed to the claims website and begin inputting your claim.
2) Filing Via Mail Service -- A package is available at the base legal office or on-line to file a paper claim. After reading and completing the paperwork, mail the claim to: Air Force Claims Service Center, 1050 Forrer Blvd, Dayton, Ohio 45420. However, people should remember that filing a claim via regular mail will take longer due to mail delays and the time to manually enter it into the system. Delays can be longer from overseas.
The pink form, DD Form 1840/1840R, entitled "Joint Statement of Loss or Damage at Delivery/ Notice of Loss and/or Damage" needs to be submitted one of three ways. 1) Directly through the carrier, 2) Upload through the AFCSC website, or 3) You can bring it into the Legal Office to have it uploaded.
