Health care services provided by the Defense Department are available to you and your family at your installation. We know that finding the right health care is important, especially in special circumstances such as emergency or during recent move. The Naval Hospital Jacksonville is a general medical and surgical hospital offering both inpatient and outpatient care. The hospital also has the Navy's largest Family Practice Training Program. Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.
Filling prescriptions while traveling
You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.
Getting dental care while traveling
Getting dental care while traveling depends on your location and whether you are a service member or family member.
With permanent change of station orders, ask for a copy of your medical and dental records from your military treatment facility and the dental treatment facility. Do this at least one month before your PCS date. The MTF should also transfer a copy of your record and any family records to your new duty station or you may be able to hand carry them to your new duty station.
If you want someone else to be able to get medical or dental information on you or your family while you are moving, you need to complete a DD Form 2870, "Authorization for Disclosure of Medical or Dental Information."

Naval Hospital Jacksonville’s priority is to heal our nation’s heroes — sailors, Marines, soldiers, airmen, guardsmen (active and retired) — and their families. The command includes a hospital and five branch health clinics across Florida and Georgia.
Branch Health Clinic Jacksonville is devoted exclusively to active-duty care, and is at Building 964 (on Ajax Street between Enterprise and Birmingham Avenue).
If you have a primary care manager at our hospital or at a branch health clinic, you’re part of a medical home port team of caregivers. Led by the primary care managers, each team focuses on meeting your preventive, routine and urgent needs.
Convenience options include the 24 hours a day/seven days a week nurse advice line, the TRICARE Online patient portal, secure email messaging and pharmacy home delivery.
Naval Hospital Jacksonville consistently earns The Joint Commission’s Gold Seal of Approval, and our medical home port teams have earned Primary Care Medical Home Certification from The Joint Commission. The hospital was the first on Florida’s First Coast to earn baby friendly certification from Baby Friendly USA.

To reschedule or cancel an appointment, please call ahead. We recommend 24 hours in advance. This allows us to give your time slot to another patient who needs care.
Your medical home port team is focused on your total health and wellness, so it’s better positioned than an emergency room to provide urgent, routine and preventive care. In a medical emergency, call 911 or go to the nearest emergency room. The branch health clinic doesn’t have an emergency room.
