For military service members in the Battle Creek, Michigan area, Grace Health is the primary health care which serves eligible military beneficiaries in Battle Creek. Priority for health care services is given to Active Duty personnel and Tricare Prime enrollees. Personnel enrolled in TRICARE Standard are seen on a space-available basis. Please note that TRICARE Standard/Extra are the primary health care for non-active duty enrollees and beneficiaries. For all Tricare info go to www.TRICARE.mil
When you move, TRICARE moves with you. No matter where you go in the world, TRICARE is there before, during and when you get to your next duty station.
There are steps you need to take to make sure changes to your TRICARE coverage go smoothly. Understand that when you move, you may have to use a different TRICARE option. Visit TRICARE Moving to learn more.
Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.
Filling prescriptions while traveling
You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.
Getting dental care while traveling
Getting dental care while traveling depends on your location and whether you are a service member or family member.
With permanent change of station orders, ask for a copy of your medical and dental records from your military treatment facility and the dental treatment facility. Do this at least one month before your PCS date. The MTF should also transfer a copy of your record and any family records to your new duty station or you may be able to hand carry them to your new duty station.
If you want someone else to be able to get medical or dental information on you or your family while you are moving, you need to complete a DD Form 2870, "Authorization for Disclosure of Medical or Dental Information."
For more information on getting copies of medical records, visit TRICARE Request Copies of Medical Records.
Excellent medical services are available in the Battle Creek area.
Medical Care
For military service members in the area, the Battle Creek Family Health Center is the primary health care provider which serves eligible military beneficiaries enrolled in TRICARE Prime Remote. In addition to the Family Health Center, the Bronson Battle Creek Hospital is the local hospital that provides specialty care treatment to the Active Duty personnel.
Priority for health care services is given to Active Duty personnel and TRICARE Prime enrollees. TRICARE Extra/Standard is the TRICARE option for Battle Creek and surrounding areas. TRICARE Prime providers are a minimum of one hour drive. Personnel enrolled in Tricare Standard are seen on a space-available basis.
TRICARE
Your key to TRICARE eligibility depends on your enrollment in the Defense Enrollment Eligibility Reporting System (DEERS). The DEERS record will indicate the dates of eligibility. All uniformed services sponsors (active, reserve or retired) should ensure that their family status (marriage, death, divorce, new child, etc.) and residential address are current in DEERS at all times. DEERS enrollment and/or updates are completed at uniformed services personnel offices, not TRICARE service centers.
For more information about DEERS, contact the Defense Manpower Data Center Support Office (DSO) Telephone Center from 6 a.m. to 5 p.m., Pacific Time, Monday through Friday, at the following toll-free number: 1-800-538-9552 or visit the TRICARE Military Health System website.
