Health care services provided by the Defense Department are available to you and your family at your installation. We know that finding the right health care is important, especially in special circumstances such as emergency or during recent move. Here, you’ll find information and options about the services you have regarding healthcare.
The 5th Medical Group is an integrated health care delivery system that provides services to active duty members, retirees, and their families. Any services not provided in the Medical Treatment Facility (MTF) will be acquired from providers in our local network. When you move, TRICARE moves with you. No matter where you go in the world, TRICARE is there before, during and when you get to your next duty station.
There are steps you need to take to make sure changes to your TRICARE coverage go smoothly. Understand that when you move, you may have to use a different TRICARE option. Visit TRICARE Moving to learn more.
Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.
Filling prescriptions while traveling
You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.
Getting dental care while traveling
Getting dental care while traveling depends on your location and whether you are a service member or family member.
With permanent change of station orders, ask for a copy of your medical and dental records from your military treatment facility and the dental treatment facility. Do this at least one month before your PCS date. The MTF should also transfer a copy of your record and any family records to your new duty station or you may be able to hand carry them to your new duty station.
If you want someone else to be able to get medical or dental information on you or your family while you are moving, you need to complete a DD Form 2870, "Authorization for Disclosure of Medical or Dental Information."

Medical Care
The 5th Medical Group is an integrated health care delivery system that provides comprehensive services for the military community. Active duty members, retirees, and their families will find that the 5th Med Group strives to provide high quality, cost effective care in a timely manner.
Any services not provided in the Medical Treatment Facility will be acquired from providers in our local network. Active duty personnel are guaranteed access to a 5th Medical Group health care provider within 24 hours for acute illnesses, seven days a week. The 5th Medical Group adheres to a set of universally accepted standards that apply to all Tricare Prime enrollees, which includes acute care access in 24 hours, routine access in 7 days, and specialty care in 30 days. Beneficiaries will find a full range of services including:
Beneficiaries will enroll in TRICARE Prime at the Tricare Service Center located on the third floor of the MTF, or during Base In processing/Medical Right Start. Each member will be assigned to a Primary Care Management team which is designed to fully meet the health care needs of their population. Each team consists of a Provider, Nurse, two Medical Technicians and administrative support. Our goal is to have beneficiaries know their Provider by name to ensure the best continuity of care.
Appointments can be made by calling the central scheduling Tricare number.
The VA Clinic is also located on the first floor of the hospital.
Health and Wellness Center
The HAWC is located on the 3rd floor of the Medical Group. Active Duty families and retirees can obtain information and guidance on nutrition, fitness, tobacco cessation, stress management, and other health related topics. Call 312-701-723-5442 for the Exercise Physiologist, 312-701-723-5399 for the Health Promotion Manager, and 312-701-723-5274 for the Dietician.
Dependent Dental Care
The base dental clinic is for Active duty only. Dependents can be enrolled in the TRICARE dental insurance program, MetLife, for use in the local community. To enroll, you must go to the TRICARE office on the third floor of the base hospital.
Special Needs
Family Advocacy is responsible for the Special Needs Identifier and Relocations Assistance Program. This program is designated to ensure service availability for family members of active duty military in the event of a PCS. It identifies exceptional needs of family members (medical, mental health, and educational). Family Advocacy verifies resource availability in both military and civilian communities. Enrollment is mandatory for all active duty military personnel with a family member with an exceptional need.
Special Needs Programming
The Minot AFB Military and Family Readiness Center (M&FRC) staff can assist with on/off base information and referral services for military with special needs family members. We understand that parents are their child’s best asset and we are here to support each family as they work in the best interest of their child. The M&FRC staff can provide families with material to learn more about a family member’s condition, help families to explore their options, provide links to a variety of local/state/national service agencies, and bring training that will help families become more knowledgeable thus stronger parent advocates.
