Health care services provided by the Defense Department are available to you and your family at your installation. We know that finding the right health care is important, especially in special circumstances such as emergency or during recent move. Here, you’ll find information and options about the services you have regarding healthcare.
Naval Health Clinic Lemoore is located in the heart of Central California. It is a nationally recognized, patient-centered medical home port clinic. As an enrollee, you will be cared for by highly qualified teams of professional medical practitioners who will ensure that you receive the outstanding care you deserve.
Enrollment to the NHCL medical home model is done via phone by calling Health Net Federal Services at 844-866-9378 or going online at the TRICARE West website.

When you move, TRICARE moves with you. No matter where you go in the world, TRICARE is there before, during and when you get to your next duty station.
There are steps you need to take to make sure changes to your TRICARE coverage go smoothly. Understand that when you move, you may have to use a different TRICARE option. Visit TRICARE Moving to learn more.
Before you move, you should take care of any routine medical needs, including immunizations, and fill your prescriptions so you have enough while traveling. If you need care during your move, visit TRICARE Getting Care When Traveling to learn what to do.
Filling prescriptions while traveling
You should refill prescriptions before traveling. But if you run out of a prescription drug while traveling, visit TRICARE Filling Prescriptions When Traveling.
Getting dental care while traveling
Getting dental care while traveling depends on your location and whether you are a service member or family member.
With permanent change of station orders, ask for a copy of your medical and dental records from your military treatment facility and the dental treatment facility. Do this at least one month before your PCS date. The MTF should also transfer a copy of your record and any family records to your new duty station or you may be able to hand carry them to your new duty station.
If you want someone else to be able to get medical or dental information on you or your family while you are moving, you need to complete a DD Form 2870, "Authorization for Disclosure of Medical or Dental Information."

Appointments
Routine appointments can be made with central appointments at 559-998-0889, 7:30 a.m.-4:30 pm, Monday-Friday. The central appointments office is closed on weekends.
Emergency Care
If your medical condition is life-threatening but not limited to chest pain, difficulty breathing and serious trauma, seek care at the nearest emergency department by calling 911. No preauthorization is required. If you are admitted into a civilian hospital, you or a family member is required to contact our hospital within 24 hours or as soon as medically reasonable.
Urgent Care
The NHCL urgent care clinic provides all beneficiaries access to acute, non-emergent, non-obstetric medical care. This includes evaluation and treatment of medical, minor surgical and mental health conditions to patients of all ages. NHCL offers pharmacy, laboratory and limited radiologic (plain film and non-contrast CT) services on-site to enhance the diagnostic and treatment capabilities of the urgent care clinic.

The Nurse Advice Line
The nurse advice line is available 24 hours a day/seven days a week for medical advice. The NAL consists of a team of registered nurses who are available to answer a variety of urgent health care questions. They can help you decide whether or not to see a health care provider. To access the NAL, dial 800-TRICARE (874-2273), Option 1.
Records
NHCL keeps and maintains outpatient records. To avoid lost medical information, please request a copy of civilian medical records to be sent to NHCL following any care received outside of the base clinic.
Photocopies of your medical records can be provided. Please allow at least one week’s notice to allow time to process the copies.
Pharmacy Services
NHCL offers several different options for obtaining your prescriptions.

Maternity Care
Obstetrical deliveries and gynecologic procedures requiring inpatient stays are performed by NHCL providers via an external resource sharing agreement with Adventist Medical Center-Hanford.
Wellness Center
The wellness center offers programs on nutrition education, weight management, meal planning, diabetes meal planning, cholesterol education, fitness assessments, tobacco cessation and a nutritional information library. Call 559-998-4584 for additional information on these programs.
Note: Due to the COVID-19 pandemic, many wellness center programs may be limited.
Immunization Clinic
Dental Clinic
The main dental clinic is located in Building 926 just south of the hospital, and the operations side dental clinic is located in Building 11 on the operations side of the base. Comprehensive dental care services are provided for active-duty military members. Specialty care in oral surgery, periodontics, and prosthodontics is available.
Normal clinic hours of operation are Monday-Friday, 7:30 a.m.-4 p.m. Routine examinations and appointments for treatment may be scheduled by calling main dental clinic at 559-998-4220 or the operations side dental clinic at 559-998-4300.
Substance Abuse Rehabilitation Program
The Substance Abuse Rehabilitation Program is located in NHCL. It provides all commands with a program of clinical screening, evaluation, referral and therapeutic counseling for alcohol and drug abusers, supplemented by education and administrative assistance.
SARP offers a non-residential alcohol treatment program, continuing care counseling group and a 20-hour alcohol impact course. Referrals to residential drug and alcohol centers are also made for those with more serious problems. Call 559-998-4474 for more information.
